Termination of a Middle-Aged Adult Instructions:
1. Record in your contact notes a termination interview, indicating the reason for termination and any follow-up arrangements you made for the client after he leaves your services. Refer to the goals for such an interview in this chapter.
2. Write your client a letter as a follow-up to this interview, summarizing for the client the discussion and resolutions that took place in your last contact.
3. Prepare a discharge summary on your client. It will be the last item in the clientâ€™s chart. Use the guidelines for a discharge summary found in this chapter
